What is C & TC?

C&TC is a program that anyone 0-20 years of age on Medical Assistance (MA) in Minnesota is automatically enrolled in. We have staff available to help you find a provider if you don't have one. When you see the provider at the clinic, you will get a Child & Teen Checkups exam. It is just like an annual physical exam, but with a few additional screening pieces. Child & Teen Checkup services include:

  • Reminder letters for periodic well child exams.
  • Linkage to medical, dental, and vision providers.
  • Assistance making appointments for well child exams and follow-up appointments.
  • Assistance with transportation to well child exams.
  • Education to parents on the importance of well child exams.
  • Coordination with family home visitors, Head Start, social workers, and WIC.

  • Information about physical and mental health.
  • Complete head-to-toe physical exam.
  • Immunizations, when needed.
  • Hearing check.
  • Vision check.
  • Blood Lead/Lab Tests (when needed).
  • Checks on development and growth.
  • Oral health screening and a referral to the dentist.
  • Time to ask questions and get answers about your child's health, behavior and development.
  • Time to discuss your child's thoughts, feelings and relationships.

Newborn - 11 months

  • Schedule newborn visit, then at ages 2, 4, 6, 9 and 12 months
  • A check on how your baby is growing, learning, developing, and feeling
  • Height and weight
  • Vaccines
  • Learn healthy dental habits
  • Mother wellness: baby blues and postpartum depression screening
  • Get answers to your questions


12 Months - 2 Years

  • A check on how your toddler is growing, learning, developing, and feeling
  • Height and weight
  • Blood Tests/Blood Lead
  • Vaccines
  • Tooth fluoride
  • Get answers to your questions


3 Years - 5 Years

  • A check on how your child is growing, learning, developing, and feeling
  • Vision and hearing check
  • Vaccines
  • Tooth fluoride
  • Get answers to your questions


6 Years - 10 Years

  • A check on how your child is growing, learning, developing, and feeling
  • Vision and hearing check
  • Healthy habits
  • Sleep, diet, screen time, safety
  • Get answers to your questions


11 Years - 17 Years

  • Checkups every year to catch problems early
  • One-on-one time with the provider to encourage a child's responsibility for their own health
  • Vaccines
  • A chance to check in about sexual health and relationships, stress and coping
  • May be combined with a sports physical
  • Get answers to your questions


18 Years - 20 Years

  • Catch-up on vision, hearing, dental and immunizations
  • A checkup on physical health, mental health, sexual health and relationships, stress and coping, and substance use
  • Get answers to your questions
